The official Ralphs survey page offers two options. Choose the one that matches your invitation:
- Start Grocery Survey: for regular store visits, with an invitation on your grocery receipt.
- Start Pharmacy Survey: for visits to the Ralphs pharmacy, with an invitation from your pharmacy purchase.
What You Need Before You Start

- Your Ralphs Rewards card number or the phone number linked to it, so your fuel points go to the right account
